Services Offered

 

Our services are available 24 hours a day, 7 days a week.

 

· Visits by trained hospice nurses, social workers, chaplains, home health aides, volunteers and therapists, as needed

· Social workers who can provide emotional support to the patient and their family members

· Coordination of community services and resources

· Advanced symptom control and pain management methods

· Therapy services for relief of symptoms, as needed

· Trained volunteers to provide patient with companionship, support and respite for the caregiver

· Education and bereavement support for families and caregivers

 

Hospice Care Team

 

 

The interdisciplinary hospice team works with the patient and their family to create a personalized plan of care for both patients and their families. The plan of care will address physical, emotional and spiritual needs with a focus on comfort.

 

Depending upon the unique and individual needs of the patient, hospice care team members typically include:
